Under minimal supervision, the Director of Clinical Services provides clinical oversight, regulatory support, and applies appropriate resources to the development, implementation, and administration of clinical services through direct communication and partnership with the Healthcare and Quality team, community leadership, and VP of Operations.  S/he will collaborate across the organization and with external partners, utilizing a systematic, comprehensive, data-driven approach to achieve clinical quality goals and improve overall resident care standards and health outcomes. Other key duties and responsibilities may include, but not necessarily be limited to:

  • Assessing, identifying, recommending and administering, in concert with community management, clinical performance standards and benchmarks that assurance the delivery of person-centered, quality clinical services.
  • Conducting on-site reviews of healthcare services operations, clinical records, consultant reports, and staff performance.
  • Providing consultative services, direction and support in the identification, development and implementation of clinical programs; processes and procedures that support community clinical services management in the efficient and effective delivery of exceptional quality care; and reports of consultation activities including findings, recommendations and actions plans.
  • Monitoring and analyzing internal quality measures, clinical outcome data and trends and identifying high-risk problem areas (e.g., food/fluid intake, weight loss/gain, poly pharmacy, psychotropic medication usage, behavior monitoring, skin care/pressure ulcer, pain management, Hospice/palliative care and other indicators for significant change of condition).
  • Monitoring infection control practices, MDS accuracy, and care plan development and implementation.
  • Mentoring and assisting with the onboarding of new clinical directors (Director of Nursing, Director of Staff Development, Director of Wellness, etc.).
  • Providing clinical EHR system support to communities and monitoring clinical EHR system utilization to educate community leadership how to effectively use EHR to its full functionality.
  • Supporting the implementation, upgrades, and training for the EHR system and incorporating standardization across the system to ensure successful outcomes are met including identifying key risks, issues, and process improvement to support effective EHR system management.
  • Assessing and providing leadership, resources and education for clinical team members across care continuum and  educating end-users to accurately and efficiently use EHR system.
  • Providing resources and consultation to assist communities in providing quality person-centered care that exceeds regulatory requirements and maximizing reimbursement; overseeing Health Center MDS Resident Assessment Instrument process and Prospective Payment System compliance.
  • Providing recommendations and tools to communities to assist in preparing for state and federal surveys; monitoring communities' compliance with corporate standard of audits.
  • State and Federal Survey findings; providing assistance, input and oversight of responses to surveys.
  • Providing clinical oversight and analysis of issues related to Corporate Risk to minimize exposure.
  • Providing support to community Directors through coaching, training, maintaining standards and the allocation of employees; develops and provides training programs and manuals.
  • Supporting the implementation of new programs including EHR system by providing the necessary materials, information, guidance and structure; provides direct support to the community as requested to improve clinical services.
  • Developing and monitoring compliance with state and federal regulations to provide quality care and conform to evidence-based clinical standards; teaching and implementing new programs and practices required by regulatory agencies.
  • Assisting in the development for corporate policies and procedures and assists with the implementation at the community through clinical leadership.
  • Overseeing documentation compliance by review of clinical records and other documents such as billing/claims for completeness and accuracy to assure regulatory compliance and financial status.
  • Remaining current on legislation and regulations that may impact community operations; interfacing with government. regulatory offices as a management agent and ensures regulatory compliance; maintaining effective working relationships with representatives of State and local agencies, industry peers, and others involved in government.
  • Providing on-site interim support as needed.
  • Conducting and educating team members on Predictive and Reactive (Root Cause Analysis) Analysis techniques.

Qualifications: 

  • Graduate of an accredited nursing school and currently licensed as an RN to practice in California.
  • BA/BS degree of equivalent education, training and professional experience
  • Minimum 5 years of experience as a Director of Nursing in Long-Term Care
  • Minimum of 3 years of experience with clinical management at CCRC/RCFE
  • Minimum 3 years of clinical EHR system implementation experience as a trainer or super-user
  • Current RN License in California
  • Certified as a Residential Care Facility for the Elderly (RCFE) Administrator
  • Licensed as a California Nursing Home Administrator (NHA) preferred
  • AANAC RAC-CT preferred
