Job Description
e high-quality, patient-centered care in a Skilled Nursing Facility (SNF) setting. The RN will collaborate with an interdisciplinary team to support residents’ long-term care needs, ensuring optimal health outcomes and quality of life.
Key Responsibilities
• Assess, plan, implement, and evaluate individualized patient care plans
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes promptly
• Provide skilled nursing care, including wound care and IV therapy
• Supervise and delegate tasks to LPNs and CNAs
• Collaborate with physicians and healthcare team members
• Educate patients and families on care plans and treatments
• Maintain accurate and compliant documentation
• Follow infection control and safety protocols
• Participate in care planning and quality improvement initiatives
Qualifications
Education:
• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Licensure & Certifications:
• Active RN License (Required)
• BLS Certification (Required)
• ACLS Certification (Preferred)
Experience:
• New graduates are welcome (training provided)
• Minimum 1 year of nursing experience preferred
• SNF or long-term care experience preferred
Skills & Competencies
• Strong clinical assessment and critical thinking skills
• Knowledge of geriatric and long-term care practices
• Excellent communication and teamwork abilities
• Strong organizational and time management skills
• Detail-oriented with accurate documentation practices
• Ability to manage multiple patients effectively
• Compassionate and patient-focused approach

- • Local: $38 – $42/hour (Based on Experience)
- • Travel: $40 – $50/hour (Based on Experience)

Responsibilities
- • 3 Days per Week
- • 12-Hour Shifts (Day/Night)
- • Approximately 36 Hours per Week
