Job Description

This position offers an opportunity to work closely with diverse populations, including those with dementia, Alzheimer's disease, or requiring hospice care, while utilizing your caregiving expertise in a dynamic and rewarding environment.
Duties
• Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and transfer devices
• Monitor patient health by taking vital signs, observing changes in condition, and accurately documenting medical information within Electronic Health Record (EHR) systems while maintaining strict compliance with HIPAA regulations
• Experience on working with home health and geriatric patients
• Support patients with memory care needs by implementing tailored strategies for dementia and Alzheimer's care to ensure safety and comfort
• Assist with meal preparation and nutritional needs while maintaining cleanliness through light housekeeping tasks such as laundry and cleaning to promote a healthy environment
• Maintain detailed medical documentation of patient interactions, medication administration, and progress notes to ensure seamless communication among healthcare teams
• Engage actively in home & community care activities by fostering positive relationships with patients and their families to promote independence and emotional well-being
Experience
• Proven experience working as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home care settings
• Hands-on experience caring for individuals with developmental disabilities, dementia, Alzheimer’s disease, or those receiving hospice or long-term care services
• Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation and medical records management
• Knowledge of medical terminology relevant to patient care procedures and medication management
• Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice training are advantageous
• Demonstrated ability to work independently while following detailed care plans and instructions from healthcare professionals
• Strong interpersonal skills to work effectively with diverse populations including seniors and people with disabilities in home & community environments
