Where You'll Work

We serve patients across: McHenry, Kane, Cook, DuPage, Will, Winnebago, and Boone Counties

Field-based role — you'll travel to patients in their homes within your assigned area.

What You'll Do
Registered Nurse - Overview:
• OASIS Start of Care, Recertification and Resumption and case management
• Assessing or re-evaluating the patient's condition and needs to determine homebound status and plan of care
• Provide patient care according to patient's care plan and needs
• Teach the patient and/or family members about care and support skills
• Medication management and education for patient and/or family
• Communicate regularly with Clinical Care Coordinators, Interdisciplinary team, and overseeing physician
• Accurately and thoroughly document patient care within company policy
• Supervision of License Practical Nurse and CNAs.
• Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life.

Qualifications:
• Active Illinois RN license in good standing
• Valid driver’s license, reliable vehicle, and auto insurance
• Current CPR Certification
• Prior experience in home health, hospice, rehab, or hospital setting is required
• Familiarity with Electronic Medical Records (EMR) software
• Comfortable with technology - a company tablet will be provided
• Strong communication and interpersonal skills
• Time management to balance patient care, travel, and documentation
• Great customer service
