Job Description

• Traveling to patients’ homes and managing their care plans according to physicians’ instructions
• Maintain accurate, detailed reports and records on medical information, vital signs, or changes in patient’s conditions.
• Monitor patients' medication usage and results.
• Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes
• Evaluate patients' behavior to formulate diagnoses or assess treatments.
• Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
• Provide personal assistance, medical attention, emotional support, or other personal care to patients.
• Coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support.
• Train other Nurses, coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support.
• Assures the continuity of care through the delivery of quality patient/client care.
• Provides effective communication to patient/client/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner
• Monitors assigned cases to ensure compliance with physician orders and requirements of third party payor.
• Demonstrates commitment, professional growth, and competency.
• Other duties as assigned.
