Job Description
Miracle Adult Day Health Care (ADHC) is seeking a Licensed Registered Nurse to join our team in San Bernardino, CA! This is an exciting opportunity for a motivated Registered Nurse who is looking for a soft nursing career with a growing organization. We provide services to help individuals with developmental disabilities develop their social and emotional skills and are a valued part of the community. We are looking for someone who wants to make a difference in the lives of our clients and their family members.
Purpose
In order to improve or maintain participants health status, nursing services will provide:
Responsibilities include but not limited to:
• Conduct an initial assessment, quarterly, and reassessments to determine skilled nursing needs and need for personal care and assistance in ADDLs. Reassessments are required to be performed every six months but may be performed anytime there is an absence of more than 30 days due to an illness or change in condition or health.
• Develop a plan of care to meet the participants nursing needs and set long- and short-term goals taking into consideration to the stated wants needs and goals of the participant, their caregivers and/or family members.
• Provide skilled supportive and restorative care as outlined on the IPC. Document all treatments and interventions on the same day the services are provided.
• Act as liaison with the participant's physician. Responsible obtaining the H&P from the primary care physician. Responsible for ensuring that the information on the H&P is complete.
• Provide health education and counseling to participants and family, if indicated.
• Be responsible for the administration of medication.

Qualifications
Must be bilingual- Spanish
• Nursing services are provided by a registered nurse licensed to practice in California.
Registered Nurse (RN) is available during all hours of operation.
Part time 6 hours a day Monday-Friday 8:30-2:30 PM *some Saturdays *
