Job Description

Active CCSS Certification (REQUIRED) Valid Driver's License Reliable vehicle (required for community-based work) Experience in behavioral health, social services, case management, or related field preferred but not required Strong documentation, communication, and time-management skills Ability to work independently and manage a flexible schedule Job Duties & Responsibilities:
Provide direct community-based support to adults with behavioral health needs Assist clients with independent living skills (budgeting, transportation, scheduling appointments, etc.) Support clients with housing stability, employment goals, and access to community resources Coordinate care with therapists, medical providers, case managers, and community partners Assist clients in navigating benefits such as Medicaid, food assistance, and insurance portals Help clients develop healthy routines and coping skills Complete accurate and timely progress notes in compliance with CCSS standards Advocate for clients and promote long-term self-sufficiency This role requires travel within the community and maintaining professional boundaries at all times.
