Job Description

The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
Schedule:
Per diem; 2 days a week or 6 shifts per month; 11:00am - 7:00pm
Location:
San Mateo County with occasional travel to the city of San Ramon
Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.
Responsibilities
• Make in-person nursing visits to patients as assigned, or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
• Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.
Requirements
• One year experience as a professional nurse in the past three years or Bachelor’s degree in nursing from an accredited School of Nursing.
• Current license as a registered professional nurse in the State of California.
Nice-to-haves
• Experience in acute care/ home health and/or hospice setting preferred.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.
