Job Summary
We are seeking a motivated and passionate Per Diem Physical Therapist to join our team in treating patients in Home Care throughout Brooklyn. As a Physical Therapist at Metro, you will have the opportunity to work with a progressive, supportive, and innovative team that values patient care and professional growth.
Key Responsibilities
• Develop and implement individually designed programs to restore function and independence in the home setting
• Evaluate patient needs and develop treatment plans in consultation with physicians, families, and patients
• Provide timely and accurate documentation of visits and communicate effectively with family members and other team members
• Collaborate with other healthcare professionals to ensure seamless patient care
Requirements
• Graduate of a Physical Therapist program
• Current NY Physical Therapist License or eligibility
• Ability to evaluate patient needs and develop effective treatment plans
• Excellent communication and interpersonal skills
• Certifications in CPR/First Aid required
