- LPN will be paired with a RN or UNL to assist with the patient load.
- LPN tasks may include oral meds, IV piggyback meds, trach care, dressing changes, wound care, hourly rounding, reinforcing patient education and completion of discharges.
- LPNs will document assessments and care plans after the RN has completed the initial assessment and initiated the care plan.
- Administers medications, including IV piggybacks and fluids but excluding IV Push medications
- Documents assessments on assigned patients, given that an RN has charted the initial assessment
- Charts on care plans, which the RN has opened; will have a RN open any new care plans
- Communicates with providers to receive orders and coordinate care for patient
- Completes dressing changes and wound care
- Completes trach care
- Inserts NG tubes and maintains feedings through NG tube
- Cares for and changes ostomies
- Reinforces education and discharge instructions
