Job Description
As a home health physical therapist, you will have the opportunity to work one-on-one with patients to help them achieve their goals and improve their overall quality of life. Your responsibilities will include conducting assessments, creating personalized care plans, and providing ongoing support and education to patients and families.
You will work closely with other healthcare professionals, including nurses, occupational therapists, and social workers, to coordinate care and ensure that patients receive comprehensive support services.
