Job Summary
This role is responsible for coordinating pre-admission and admission processes, ensuring a smooth transition for patients and their families.
You will:
• Obtain pertinent information from patients and/or family members, referral sources, and centralized intake for admission and complete admission paperwork.
• Verify that patient rooms are ready prior to admission.
• Sustain contact and provide support to patient/families during their transition.
• Orient patients to the center environment.
• Participate in interdisciplinary teams to assist with healthcare navigation needs.

Requirements
To be successful in this role, you will need:

• Associate's degree in related field with three years experience
