Role of the Community Health Nurse (RN):

The Registered Nurse is responsible for organizing and delivering nursing care to residents in need of long-term and rehabilitative care.

Major Responsibilities:

  • Adhere to the Nurse Practice Act and facility guidelines while exercising nursing judgment.
  • Assess, plan, and evaluate nursing care for patients requiring long-term and rehabilitation support.
  • Implement care plans and assess patient outcomes.
  • Supervise the nursing staff in their patient care duties.
  • Contribute to the formulation and evaluation of care standards.
  • Maintain up-to-date knowledge of documentation requirements and equipment operation.
  • Ensure confidentiality and respect patient rights.
  • Provide educational support to patients and their families as needed.
  • Conduct oneself professionally per facility policies.
  • Work rotating shifts, including weekends and holidays as required.

Qualifications Needed:

  • Must be a graduate of an accredited nursing program.
  • At least one year of medical/surgical nursing experience, ideally in acute or long-term care settings.
  • Valid RN license in the state(s) of practice and reliable transportation required.
