The Community Health Program Manager plays a crucial role in leading strategic health improvement efforts within our communities. This position is responsible for ensuring compliance with IRS Community Benefit requirements and executing comprehensive community health strategies. The Program Manager serves as a content expert and drives initiatives related to the Community Health Needs Assessment (CHNA), Community Health Plan development, and ongoing Community Benefit tracking. This role involves leading the CHNA process, ensuring adherence to IRS regulations, and identifying best practices to improve community health outcomes. Responsibilities also include collecting and recording community benefit activities, collaborating with the Finance department, and actively participating in customer service initiatives.

Detailed Duties and Job Responsibilities:
• Analyze the healthcare environment to identify and lead opportunities for advancing community benefit initiatives within the assigned coverage area.
• Advocate for health improvement efforts through policy, systemic, and environmental changes.
• Ensure timely and accurate responses and follow-up to inquiries and requests.

Campus Engagement:
• Collaborate with campus leaders to create and execute CHNA, Community Health Plans (CHP), and annual CHP evaluations.
• Serve as the primary point of contact for campus updates on progress and ensure community benefit is integrated into strategic and financial planning.
• Inform executives on Community Health initiatives, progress, and new requests.
• Ensure campus-specific programs are executed effectively.

Program Implementation:
• Collect metrics for strategic health programs within the assigned coverage area.
• Support effective reporting of community benefit initiatives.
• Activate and manage various strategic health programs.

Community Engagement:
• Establish and grow relationships with key partners and organizational leaders.
• Lead collaborative teams for CHNA and CHP development.
• Partner with Community Relations and attend community events.

Community Health Needs Assessment:
• Provide strategic direction for the CHNA process.
• Identify best practices to improve community health.
• Analyze data to inform program planning.
• Serve as the point of contact for Corporate.

Knowledge and Skills Required:
• Strong organizational, analytical, and problem-solving skills.
• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
• Excellent communication and interpersonal skills.
• Strong leadership and team-building abilities.
• Ability to manage multiple projects and priorities effectively.
• Ability to conduct research, prepare comprehensive reports, and implement successful strategies.
• Ability to read, analyze, and report on financial statements and program reports.

Education and Experience Required:
• Bachelor’s degree in Public Health, Public Policy, Healthcare Administration, Business, Non-profit Management, or a related field.
• Two or more years of project management experience in healthcare, business, government, or a community organization.
• Experience in budgeting, financial analysis, health programming, and community events.
• Experience working with healthcare and/or non-profit industry leaders.
• Experience working with community and government leaders.
