**Bereavement Counselor –** **AdventHealth Hospice**

• *Schedule:** Full time
• *Shift** : Monday - Friday 8:30am to 5pm. Could have occasional evening group hours.
Location: Altamonte Springs Hospice Office
• *The role you’ll contribute:**
The Bereavement Counselor is responsible for providing bereavement support to family members who have lost a loved one and also responsible for educating the community and serving as a community representative.
• *The value you’ll bring to the team:**
+ Provides direct services to families who have suffered significant losses (80%) * Provides bereavement support and grief education for individuals who have lost a family member and leads support groups, promotes intra-group discussions, and encourages identification and expression of feelings in a safe environment. * Contacts and assesses all family members of patients within 18 days of the patient’s death. * Offers brief therapy specifically related to grief issues of families to assist clients in developing support systems and coping skills. Informs clients of available community resources to empower clients to seek support in the community once the six bereavement counseling sessions have concluded or if the client’s needs exceed what can be provided in short term bereavement counseling. * Provides individual and group support for both hospice and community clients in the office and the field and travel as needed to participate in designated support groups and social activities. * Supports development of educational programs for the community, as assigned by manager. * Oversee volunteer calls and visits to bereaved families as well as documentation of such visits to be included in client bereavement files. * Prepares, plans and conducts bereavement training programs for volunteers who provide family visitation and phone contact of the bereaved families as well as documentation of such visits to be included in patient bereavement files. * Participates in interdisciplinary teamwork with nurses, chaplains, social workers, and Patient Care Manager and collaborates with the team to help identify the needs of bereaved family members and assesses high-risk or immediate grief-related needs. * Participates in community outreach efforts such as community fairs, conferences, and symposiums and develop presentations for in-house and community audiences.
+ Additional Duties and Responsibilities: (15%) * Participates in staff meetings, department meetings, in services and other related activities. * Attends educational and developmental seminars and meetings, as deemed appropriate. * Develops recreational and social activities for the bereaved. * Arranges for the expansion of bereavement services to special groups as census indicates and staffing permits. * Assists in the planning and participates as a representative at the bi-annual Memorial Services. * Collaborates with the team social worker, as needed. * Document activities; bereavement notes, session notes, relevant research, communications, correspondences, and the date and time of activities into designated patient database system.
+ Performs all other duties as deemed necessary by the Director of Human Resources or designee. (5%)
• *The expertise and experiences you’ll need to succeed** **:**
• *Minimum qualifications:**
+ Master's Degree in Clinical Social Work, Marriage and Family Therapy, Mental Health Counseling, or related field
+ An equivalent combination of education and experience may be substituted at the discretion of the Administrator
+ 3 years experience in counseling/social work or bereavement
+ Valid in state Driver’s License with current car insurance
• *Preferred qualifications:**
+ Professional license or license eligibility under Florida law
