The Opportunity:

Provides clerical support to the Health Information Management department in the areas of data capture, record analysis, record completion, birth registry, system support, quality and productivity. Ensures the integrity of all aspects of the legal health record by completing tasks in a timely fashion with exceptional quality.

The Document Imaging Specialist, under the leadership of the HIM Operations Scanning Supervisor, will perform all aspects of the Document Imaging functions for the legal medical record. The Specialist will make rounds throughout the hospital to retrieve records from patient care areas, applying accounting procedures to ensure collection of all records. The Specialist will follow accurate document preparation procedures, ensuring all documents belong to the correct patient, and identify documents that do not require scanning. The Specialist will maintain scanning equipment to ensure clarity of images, and scan documents according to procedures. After scanning the documents, the Specialist will select the correct Document Type for any documents that were not auto-indexed, understanding the document will not be able to be located by users of the medical record if it has the incorrect Document Type. The Specialist will support quality assurance by auditing scanned batches to verify documents belong to the correct patient, ensure clarity of image, and that the correct Doc Types were assigned. The Document Imaging Specialist will perform all tasks within defined expectations for productivity and quality, to ensure the integrity of the legal medical record. Keeps all patient information confidential and abides with Privacy and Security regulations.


  • Collects and prepares patient records for scanning; ensures that all documents are scanned into the correct patient file; ensures that all documents contain proper patient identification; arranges documents in chronological order and assigns batch labels to documents per HIM prepping policy and procedure.

  • Indexes documents by location and document type. Maintains record destruction log. Files, maintains and destroys paper records post imaging in accordance with policy and procedures.

  • Analyzes patient records to ensure completeness per Rules and Regulations, By-Laws, governing bodies and policy and procedures; refers incomplete records to HIM Record Completion Specialists for follow up.

  • Ensures that all birth worksheets are available and completed for all newborns; interviews mothers and obtains signatures of obstetricians and parents as needed; submits birth certificates according to standards set by the state Department of Health within ten days of birth with a 99% accuracy rate.

  • Reviews physician orders and dictations and verifies physician deficiencies within patient charts; notifies physicians of delinquencies and assists physicians with navigating HIM systems to correct the issues. Reports physician delinquency correction non-compliance to proper administrative managers; notifies appropriate site personnel of physicians' suspension of new patient admission and surgery scheduling privileges; notifies appropriate site personnel of physicians' reinstatement of privileges once compliance is reached.

Health Information Management /Medial Coding

Other Preferred Knowledge, Skills, and Abilities:

  • Detail-oriented, organized, excellent communication skills, ability to effectively communicate with physicians and their staff; ability to work in a fast-paced environment, MS Office knowledge.

  • Previous experience with EPIC and One Content preferred
