Job Summary:
We are seeking a dedicated Nurse Case Manager to coordinate and oversee skilled nursing homecare services for geriatric patients. This role involves developing care plans, coordinating with interdisciplinary teams, and ensuring optimal patient outcomes through effective case management. The ideal candidate is a registered nurse (RN) with experience in geriatric or home health care and a passion for improving the quality of life for elderly patients.
Key Responsibilities:
• Patient Assessment & Care Planning:
• Conduct initial and ongoing assessments of patients in their homes.
• Develop and implement individualized care plans tailored to patients’ medical needs and personal preferences.
• Collaborate with physicians, therapists, social workers, and caregivers to ensure comprehensive patient care.
• Case Management & Coordination:
• Serve as the primary point of contact for patients, families, and healthcare providers.
• Coordinate services such as skilled nursing, physical therapy, occupational therapy, and other medical interventions.
• Monitor patient progress and adjust care plans as needed to achieve the best outcomes.
• Patient & Family Education:
• Educate patients and their families on disease management, medication administration, and home safety.
• Provide guidance on chronic condition management, wound care, and mobility assistance.
• Compliance & Documentation:
• Maintain accurate and timely electronic health records (EHR).
• Ensure compliance with Medicare and other regulatory requirements.
• Participate in quality assurance and improvement initiatives.
Qualifications:
• Active Registered Nurse (RN) license in Florida
• 2+ years of nursing experience, preferably in home health, geriatrics, or case management.
• Strong understanding of Medicare regulations and home health care guidelines.
• Experience with electronic medical records (EMR) and care documentation.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work independently and travel to patient homes as needed.
Preferred Qualifications:
• Experience with chronic disease management, palliative care, or wound care.
• Familiarity with home health agency operations and insurance authorizations.
