Rehabilitation Healthcare Professional

BAYADA Home Health Care is a leading provider of home health care services, and we are currently seeking a Rehabilitation Healthcare Professional to join our team in Jacksonville, FL.

This individual will work closely with adult and geriatric clients on a per visit basis, providing physical therapy services to promote their comfort, independence, and dignity. As a Rehabilitation Healthcare Professional, you will be responsible for making home visits, performing diagnostic tests, implementing individualized care plans, and educating clients and their families on rehabilitative care and activities.

Requirements for this role include a current FL Physical Therapist license, a minimum of one year of recent work experience as a Physical Therapist, graduation from a program approved by the American Physical Therapy Association, and strong interpersonal, organizational, and problem-solving skills.
